Dosis pengganti hormon tiroid

Dokter menetapkan dosis Tyhrax, Euthyrox atau obat pengganti hormon tiroid lainnya sesuai dengan banyak sedikitnya kegiatan kerja dan aktifitas keseharian si pasien.

Dosis yang ditetapkan dokterku adalah satu setengah. Ketika aku ngantor setiap hari, dosis itu pas untukku.

Nah, dua bulan terakhir ini aku lebih banyak kerja dari rumah. Aku tetap memakai dosis satu setengah karena ku pikir aktifitas fisik tidak banyak berkurang.

Tetapi sebulan setelah aku kerja di rumah, aku mulai sering mengeluh kepanasan. Mudah sekali berkeringat macamnya habis berlari nonstop keliling lapangan bola atau aerobik heboh satu jam penuh, padahal aku cuma nyetir BSD-Proklamasi (ya lumayan jauh, tapi kan gerakan tubuh terbatas)atau jalan2 di pasar.

Di kamar tidur, ketika suami dan anak udah nyungsep masuk selimut, aku malah sibuk kipas2 dan memperdingin derajat si AC.

Menurut suami, mungkin aja aku keringetan gak keruan itu karena dosis obat tak cocok. Coba deh kurangi, sarannya.

Betul saudara2. Ternyata setelah aku kurangi dosis jadi 1 tablet saja, hilang tuh masalah overheating dan oversweating.

Ternyata dosis obat terlalu besar menimbulkan gejala hipertiroid: heat intolerance and excessive sweating.

Other common hypothyroidism symptoms include:
* Increased bowel movements
* Tremor (usually fine shaking)
* Nervousness or agitation
* Rapid heart rate
* Weight loss
* Fatigue
* Decreased concentration
* Irregular and scant menstrual flow (maap...lagi malas translate nih)

Sebaliknya, gejala hyperthyroidism termasuk:
* Fatigue
* Sluggishness
* Increased sensitivity to cold
* Constipation
* Pale, dry skin
* A puffy face
* Hoarse voice
* An elevated blood cholesterol level
* Unexplained weight gain
* Muscle aches, tenderness and stiffness
* Pain, stiffness or swelling in your joints
* Muscle weakness
* Heavier than normal menstrual periods
* Brittle fingernails and hair
* Depression

Thyrax sudah kembali ke pasar

Bulan Mei lalu saya menulis tentang sulitnya mendapatkan Thyrax, dan akibatnya terhadap keseharian saya.

Beberapa minggu lalu saya mendapat jawaban tentang penyebab sempat hilangnya Thyrax di pasaran Indonesia. Kebetulan saya bertemu dengan seorang pejabat Schering-Plough Indonesia yang menjelaskan bahwa beberapa waktu lalu memang sempat satu batch Thyrax yang diproduksi ternyata tidak berhasil. Memproduksi hormon bukan perkara mudah, ada saja kemungkinan produk tersebut gagal memenuhi standar yang ditetapkan sehingga tidak bisa dipasarkan. So, butuh waktu lagi untuk membuat batch yang baru. "Sekarang sudah ada kok, coba cari di apotek2 besar, biasanya mereka lebih cepat memasarkan," kata si Bapak.

Pak, saya sangat menghargai loh ketaatan perusahaan Bapak terhadap aturan produksi obat yang baik dan benar. Seandainya perusahaan Bapak tidak peduli terhadap kesehatan kami, para pasien, bisa saja batch Thyrax yang tidak memenuhi syarat itu dipasarkan diam2, dicampur dengan tablet2 yang lulus tes.

Betul kata si Bapak. Saya sudah bisa kembali minum Thyrax.

Kenapa sih harus Thyrax?

Kenapa ya? Gak ada alasan khusus, selain kebiasaan aja.

Ketika Thyrax gak ada, aku minum Euthyrox buatan Merck. Manfaatnya sama. Harganya tidak jauh beda.

Satu hal yg mungkin buat saya lebih suka pakai Thyrax adalah karena ia dikemas dalam botol, sedangkan Euthyrox dalam blister. Karena dosis saya satu setengah tablet per hari, maka jika saya pakai Euthyrox saya kuatir sisa setengah tablet akan rusak karena terekpos udara. Dengan Thyrax, saya cukup bagi dua si tablet lantas simpan sisanya di dalam botol. Tapi mungkin juga kekuatiran saya tidak beralasan.

On Thyrax shortage

I've been looking for Thyrax,Organon's levothyroxine sodium, since January with no luck.

Apothecaries no longer sell the pills. Some who still have the leftover supplies dared not to sell the drugs as they are mostly nearly expired. Several apothecaries, however, continued selling the nearly expired (two to four months before expiry dates) to desperate patients.

I initially was also desperate to get the drugs because i had been off medication for nearly three months (the first one and a half months were necessary prior to undertaking my half-yearly blood test, the rest were strictly because I couldn't find Thyrax anywhere!). I started to get VERY irritated by the hypothyroid symptoms. Hypothermia, or sudden drop of body heat, deprived me of my sleep. I became very sensitive to cold so i showered less often (OMG!!!!) and i wore thick fleece jacket not only when i sleep but also at work. I eat like a pig and put on five kilos in January (i eventually shed the extra two kilos last week!!!!). Worst, i grew even more lazier and lazierer, forgetful and absent-minded.

I needed my Thyrax fix, but nobody can tell me when the drugs will be available in the market, not even Organon reps (or in this case, Schering-Plough reps) in Jakarta.

I eventually resorted to Merck's Euthyrox. So far no complaints. But i still don't understand how an important (i'd call it life saving) drugs like Thyrax can disappear from apothecaries' shelf. Is it possible that Organon no longer sells it in Indonesia?

SPECT-CT can detect spread of cancer cells?

A hybrid single photon emission computed tomography-computed tomography (SPECT-CT) camera can accurately determine and locate the spread of cancer cells to nearby lymph nodes, say researchers from the University of Erlangen-Nürnberg, Germany.

Great! This means that doctors should now, I hope, be able to accurately distinguish between cancerous cells in regional lymph nodes and normal residual thyroid tissue directly after surgery.

time to see my doctor

Took my blood tests for T3, T4, TSH and Tg on Monday. The results should be ready this morning, hopefully, so i can see my doctor in the afternoon. Been wondering if the three kilos extra i gained in the past three weeks, and the frequent hypothermia, has anything to do with my not taking thyroxine since first week of December...
